Step 1: Simply ask your distributor representative to email to you a ONE YEAR USAGE REPORT in MS Excel. The usage report should automatically contain the following information:
- Manufacturer Name
- Product Description
- Manufacturer Number
- Distributor Item Number
- UOM (Unit of Measure)
- UOM Conversion (eg 100EA/BX, 10 BX/CS, 1000/CS)
- Usage (i.e., Number Bought)
- UOM Price (Price of Each Unit Purchased)
- Number of Units Purchased
- Dollar Amount Spent for Each Item
Step 2: Email the ONE YEAR USAGE REPORT along with the following information to [email protected]:
- Contact Name and Title
- Contact Email Address
- Contact Phone Number
- Facility Name
- Facility Address
- Facility Phone Number
- Distributor Rep’s Name
- Distributor Rep’s Email Address
- Distributor Rep’s Phone Number
